The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

Goal: Memphis Fast Forward is the shared vision and unprecedented action plan to create good jobs, a better-educated workforce, a safer community, a healthier citizenry and efficient government in Memphis and Shelby County.

Impact: Major violent and property crime were reduced by 22.8% and 25.5%. The pool of high-potential teacher candidates increased from 21 to 1,800 in one year. More than 15,000 jobs were created. New capital investment of $4.2 billion was generated. Local governments saved more than $75 million.

Impact: The Community Preventive Services Task Force (CPSTF) recommends exercise programs for pregnant women to reduce the development of gestational hypertension.

Goal: The goals of this program are:
- Detect school adjustment difficulties
- Prevent social and emotional problems
- Enhance learning skills

Impact: One study demonstrated that participants made significant improvements in task orientation, specifically in working more independently and completing tasks faster. In behavior control, program students showed increased coping skills and lower levels of aggressiveness and produced fewer disruptions. In assertiveness, students had improved participation in activities, were better at expressing ideas, and showed increased leadership and decreased shyness. Improvements in peer sociability included increases in the quality of peer relationships and improved social skills. Several other evaluations of the Primary Project present evidence of improved school adjustment and decreases in problem behaviors for participants.

Goal: The goal of the Promotores de la Salud de los Niños Program is to use volunteers from the Latino community to locate families in King County that are eligible for services and help families get connected to those services.

Goal: PATCH envisions a supportive environment in which all adolescents are able to reach optimal health, safety, and economic security.

MISSION: To improve adolescent health and well being by engaging, educating, and empowering youth and adults as trusted partners in care.

Impact: Health care providers and teens who participate in the PATCH program show significant improvements in knowledge, self-efficacy, and behavioral intentions to seek and provide quality sexual health care.

Goal: The goal of Saving Smiles is to improve the oral health and in turn overall health of children in Lafayette County, especially for the poor and underserved.

Impact: Most students from the 11 participating schools in Lafayette County received a screening, fluoride varnish, and oral health education, and most of the children referred for dental care were seen by a dentist.

Goal: The goal of See You in School is to improve the grades, self confidence and engagement of high school students through mentor/mentee relationships. Decreasing the dropout rates in Whatcom County is a long term goal that begins with the mentor/mentee relationship and a network inside the schools.

Goal: Shape Up SF aims to reduce health disparities by creating healthy eating and active living environments and improving physical activity and nutrition.

Impact: Shape Up SF implemented Safe Routes to School, Bayview HEAL Zone, PE4SF, Rethink Your Drink, Southeast Food Access (SEFA) Working Group, SEFA Food Guardians, and Walking Challenge, and increased physical activity and access to healthy foods in San Francisco.
