Impact: The Community Preventive Services Task Force (CPSTF) recommends tailored pharmacy-based adherence interventions for cardiovascular disease prevention. Evidence shows interventions delivered by pharmacists in community and health system pharmacies increased the proportion of patients who reported taking medications as prescribed. The CPSTF also finds these interventions are cost-effective for cardiovascular disease prevention.

Goal: The goals of the Mental Health First Aid course are to: 1) learn to identify risk factors and warning signs for mental health and addiction concerns, 2) utilize evidence-based strategies to help someone in both crisis and non-crisis situations, and 3) become a resource in your community to guide people where to turn for help.

NAMI-DuPage's yearly goal is to complete 70 training courses per year.

Impact: Mental Health First Aid helps community members become aware of signs and symptoms of someone in a mental health crisis and become a resource to mental health and behavioral health programs in their community.

Goal: The goal of the PATH Program is to improve knowledge of cardiovascular health and reduce risk factors associated with cardiovascular disease.

Goal: The goal of Planet Health is to reduce childhood obesity among middle school students using a school-based interdisciplinary intervention focused on decreasing television viewing and consumption of high-fat foods, and increasing fruit and vegetable intake and moderate and vigorous physical activity.

Impact: These results cumulatively show that Planet Health and similar programs can reduce obesity and increase healthy food consumption in girls. They also show that a finding of a reduction in number of hours watching TV for girls can also predict reduction in obesity.

Goal: The goal of the SISTERS program is to provide outreach/support and case management to pregnant and postpartum women who are on public assistance, are mandated to treatment, report having experienced more than four violent traumas and have smoked crack cocaine during their last pregnancy.
